Appointments

We would like to schedule routine appointments in advance. If your child is ill and must be seen, we will make every effort to take care of him/her as soon as possible.  We ask that you please call for an appointment first; generally we are unable to see non-emergency walk-ins. Please notify us at your earliest opportunity if it becomes necessary to change an appointment.  Established patients must be accompanied by legal parent/guardian or designated caregiver listed on the patient registration form.  If unsure who's listed on patient registration form, please print a new patient registration form and submit prior to your appointment.

Time is set aside daily if your child needs special attention. On certain days and during the winter season, we experience a large volume of telephone calls. We appreciate your patience as we take these calls in the order we receive them, depending  on the urgency.

Walk-in Patients

We will see walk-in patients for emergencies only. However, we do ask that you call for an appointment whenever possible.  We have a trained triage department always available for your questions. We welcome new patients to our practice. You may download patient forms and bring them to your first visit. Please bring your insurance card and child's immunization records with you. We always ask that any minor be accompanied to our office by a parent or legal guardian.

Cancellation Policy

Here at ZAMAN PEDIATRIC CENTER, our goal is to spend individual quality time with our patients.  When you call to schedule an appointment, that time is reserved for you.   We ask that when you book an appointment you mark it on your calendar and make every effort to keep that scheduled appointment time.

If you absolutely need to cancel an appointment, we require that you give us a 24 (twenty-four) hour advance notice.  By informing us in a timely manner of a change in your schedule, it allows for another patient to take advantage of the available time.  

In the event our office does not receive adequate, 24 hour notice, your account will be subject to a $50.00 late cancellation fee.  Payment will be required before another appointment can be made.

For HMO and POS plans, we must be listed as the primary care physicians (PCP) prior to your first visit to our office.

Payment Options

Unless other arrangements are approved by us in writing, the balance on your statement is due and payable when the statement is issued, and is past due if not by the end of the month.  We now process all checks through TeleCheck.  Returned checks will receive an overdraft charge of $30.00 per check.  All NSF checks must be picked up within 5 working days; they are not run back through your bank.  A $25.00 late fee will be added to all account balances beyond 30 days past due from the date of services.  Your Co-payment is also due at the time of your office visit.  A $10.00 late fee will be assessed if not received. We are now accepting online payments.

Missed Visits

We require a 24 hour notice if you are unable to make your appointment.  The second time a patient does not show up on time for an appointment, or cancels with less than 24 hours notice, a $50.00 fee will be charged.  This fee must be paid before a new appointment is scheduled.  Patients with three missed appointments will be asked to transfer their records to another doctor.

X-Ray & Lab Evaluations

X-rays are not done in this office. Special studies may need to be referred to a radiology office or hospital.  Blood and urine samples may be obtained for routine testing in the office. Special studies may need to be obtained in the office and sent to an outside laboratory for completion, or your child may need to be referred to sites outside our office. We will work with you to comply with the guidelines of your insurance to ensure proper reimbursement, but we ask you contact them for details on your plan.

Prescription Refills

We will be happy to refill prescriptions during our weekday office hours.  Please allow at least 24 hours for your request to be complete.  We will not accept refill request after hours or on the weekends!  Please have all request in Monday through Friday before 4:30 pm.  Please try to anticipate any refill needs for chronically used medications when your supply runs low.  Controlled drugs such as Ritalin, Adderall, Concerta etc...  They must be provided to the pharmacy in written form.

Custody Agreements

Whichever custodial parent accompanies the child will be responsible for executing the treatment plan for the child and be responsible for payment.  Joint custody means that each parent has equal access to the child's medical records. Should these issues become disruptive to our organization we will discharge the patient from our practice.

Medical Records

Medical Record requests must be in writing and received in our office at least 72 hours before the date that they are needed.  However our normal turn around time can take up to 14 days. The fee for medical records is $35.00 and up.  The fee must be paid prior to receiving the records.  Please note that Immunization Records are free. Visit our patient forms section.
